A Program of the Glenn Family Foundation Continuum of Growth Program
The Continuum of Growth Program is a comprehensive, wraparound services model supporting youth and young adults transitioning out of foster care. The program will operate within a 45-unit, single-occupancy Independent Living Facility, providing safe, stable housing while preparing residents for independent adulthood.

By integrating housing stability, individualized case management, education and training, life-skills development, and workforce pathways into a single coordinated model, the program will guide participants from instability to self-sufficiency and long-term success.

Housing Stability & Residential Support
​Residents will live in private, single occupancy apartments designed to promote dignity, autonomy, and security. On-site residential staff provide guidance, reinforce expectations, and maintain a supportive environment that mirrors independent adult living while ensuring appropriate structure.


Individualized Case Management
Each participant is paired with a dedicated Case Manager who provides trauma-informed, youth-driven, and strengths-based support. Case management is continuous and adaptive, ensuring services evolve as participants grow.
